Program Objective

To partner and collaborate with community structures and relevant stakeholders to promote and facilitate universal access and uptake of quality community healthcare services among self-employed, associations and Christian Churches/groups in LGAs across the Nigerian State

ELOHIM FOUNDATION Community Health Services Schemes (ELOFOC-HSS) is a health care financing reform (Social Community Health Insurance Scheme) which operates on the principles of solidarity, Risk sharing and pooling of resources for the provision of affordable, accessible and qualitative health care to individuals and groups in the informal sector on sustainable basis.

The informal Sector consists of the following:

Rural and semi-urban Communities, Christian Churches, Informal Private sector/Self-employed groups/Individual plans.

ELOFOC is a registered Civil Society Organisation based Community HealthCare insurance Scheme, accredited by National Health Insurance Scheme (NHIS) Abuja as a Community Health Insurance Managers/Technical facilitators.

To provide a reliable, strong and dynamic community healthcare services that is totally committed and focus to securing meaningful coverage and easy access to adequate and affordable healthcare.

To have every community person and families obtain optimal health care services and enjoy healthy living with little or no out of pocket expense.

Program Mission

Our mission is to facilitate access and affordable community healthcare financing through pooling and judicious utilization of financial resources to provide financial risk protections and cost burden sharing for community people, against high cost of healthcare, using prepayment programmes prior to their falling ill.

Program Objective

Benefit Package

Healthcare providers under the scheme shall provide the following benefits package to the contributors;

Out-patient care, including necessary consumable;
Prescribed drugs, Pharmaceutical care and diagnostic tests as contained in the National Essential Drugs List and Diagnostic Test List;
Maternity care for up to four (4) live births for every insured contributor or enrolee
Consultation with specialists, such as: physicians, Paediatricians, Obstetricians, Gynaecologists, General Surgeons, Orthopaedic Surgeons, ENT surgeons, Dental Surgeons, Radiologists, Psychiatrists, Ophthalmologists, Physiotherapists, etc., after referral by General Medical Practitioner;
Hospital care in a General Ward for a stay limited to cumulative 15 days per annum, thereafter, the beneficiary and/or the employees pays
Eye examination and care, including the provision of spectacles;
A range of prostheses (limited to artificial limbs produced in Nigeria);
Preventive dental care and pain relief (including consultation, dental health education, amalgam filling, and simple extractions).
Annual Medical check-up unrelated to illness.
